Payment Options

If your plan includes a monthly premium, payment is due by the 10th of each month. Paying your premium is simple, with three convenient payment options to choose from.

Electronic Funds Transfer (EFT)

Enjoy peace of mind with this convenient payment option that automatically withdraws your monthly premium from your chosen bank account.

To enroll in EFT, complete the Electronic Funds Transfer Form and send it to the address provided on the form.

Prefer to enroll digitally? Simply fill out this Online EFT Form.

Social Security Withdrawal

Set up effortless payments by having your Medicare premium automatically deducted from your monthly Social Security check.

To enroll in Social Security Withdrawal, complete the Social Security Withdraw Form and mail it to the address listed on the form.

Billing Statements

Prefer a physical bill? Receive a monthly statement and pay your premium by mailing a check to Trinity Health Plan of Michigan by the 10th of each month.

To enroll in monthly billing statements, contact Member Services.
