Request to Join Our Provider Network
Thank you for your interest in joining the Trinity Health Plan of Michigan provider network. Please carefully read the steps below and then complete the form to begin the process.

Submit an Application
Complete and submit the Provider Network Application form below to begin the contracting process. To reduce any delays in the process, please make sure that all information (including your email address) is correct before submitting.
Please note that completion of this form does not automatically grant participating access to the network if you don’t have an active contract with Trinity Health Plan of Michigan.

Application Review and Approval
Our team will evaluate the current need to service our membership in your area. If our provider panel for your specialty area is approved, a Network Manager will reach out with a contract.

Credentialing
After we receive your returned contract signed and panel roster, we will begin the credentialing process which can take up to 120 days. Trinity Health Plan of Michigan requires all initial credentialing and recredentialing be received through CAQH. Once your credentialing is complete, your contract will be finalized.

Contract Completion
Once all the steps above are completed successfully, your Network Manager will return your fully executed contract with your effective date. You will also receive welcome materials, which is when you officially become part of the network.
