The Annual Wellness Visit (AWV) is one of the most important features of our many free preventive services,1 as it establishes a comprehensive assessment of your current health and well-being. The AWV also provides the opportunity to create a personalized plan with your primary care provider (PCP) for health maintenance. The initial AWV can include:

  • A Health Risk Assessment (HRA).
  • Establishing a list of current providers and suppliers.
  • Review of your medical/family history.
  • Assess and discuss treatment options for urinary incontinence/bladder leaks.
  • Review of risk factors for depression or mood disorders.
  • Review of functional ability and level of safety.
  • Review your current medications. You can use the card below or create your own to track your medications and bring to your appointment to share with your provider.

Subsequent annual wellness visits can generally be a continuation of your initial visit. Use this as an opportunity to establish benchmarks and goals with your PCP and discuss other services or treatments for now or in the future. Take advantage of this opportunity to truly maintain your health and well-being with your PCP. Qualify for a $50 Member Reward added to your Flexible Benefit Card through the voluntary Member Reward Incentive Program.2

You can also qualify for a $50 Member Reward through a Health House Call. This is a health assessment completed by a nurse practitioner in the comfort of your home or through a virtual visit at no cost to you. Trained nurse practitioners will conduct a comprehensive care assessment and answer your health questions. Following your assessment, they will forward a summary to your PCP for review. We highly recommend that you meet with your provider to discuss the results.

Healthy House Call benefits include:

  • Members can be in the comfort of their own home.
  • There is no cost for members.
  • During the visit, the nurse practitioner will:
    • Review personal and family health history.
    • Listen to current health concerns and needs.
    • Review medications and dosages.
    • Recommend screenings or necessary diagnostic tests.
    • Suggest available healthy lifestyle resources and community services.
