The Medicare Experience Survey

If you are one of the lucky few to receive the annual Medicare Experience Survey, please take a
moment to fill it out and mail it to the return address. Or scan the QR code to fill out the survey
online! You may start to receive the survey in March and the cutoff to respond is in May. This survey is an annual questionnaire that assesses your experience with your health care coverage and services. This is your chance to give anonymous feedback to help shape your plan and benefits moving forward. Your feedback is confidential and will not be associated back to you.

• How well you were treated by your health plan’s customer service

• How easy it was to get your prescription drugs
